While replacing the upper and lower radiator hoses I discovered the CCV breather tube was broken. One of the clips holding the breather tube in place on the valve cover was broken. This was allowing the tube to pop out of place. The green o-ring was also swollen and would not allow the whole assembly to be fully inserted.

After reviewing some base line photos, the most recent I could find with the CCV tube in place was dated Feb 16, 2004. Not sure how or when the tube broke. Not sure if it was in place after last air filter change on March 15, 2005
